By completing a form and then submitting it to the U.S. Consumer Product Safety Commission, any person can report an injury or death involving consumer products. Unsafe products can also be reported to the CPSC. You can contact the Commission by mail, phone or Internet email for further details. An incident or unsafe product can be reported by calling toll-free at 1-800-638-2772 or by sending an e-mail to firstname.lastname@example.org.
The CPSC advises to keep the product involved in the incident for at least 30 days after you file this report for further evaluation by the CPSC. Also, remember the CPSC does not have jurisdiction over automobiles, trucks and motorcycles, car seats protecting children in on-road vehicles, foods, medicines, cosmetics, medical devices, or dissatisfaction with business practices.
